We've seen in the past a few players do this in the NHL, which was always met by a fine by the league, but the punishment should be more severe. The most recent example of this that this writer can think of was Alexander Ovechkin on Trent Frederic:

Ovechkin was given a minor penalty and fined $5000.
Now in a KHL game yesterday, former NHL player Yegor Korshkov was given a 5+20 min penalty for spearing, which looks to have caused serious injury.
